Your rights as a patient
- Receive care without unlawful discrimination and with respect for your dignity, privacy, personal values, and cultural needs.
- Know the identity and professional role of the individuals involved in your care.
- Receive information about your condition, proposed treatment, reasonable alternatives, expected benefits and material risks in a manner you can understand.
- Participate in decisions about your care and ask questions before agreeing to treatment.
- Refuse treatment to the extent permitted by law and be informed of reasonably foreseeable consequences of that decision.
- Have your medical information and communications handled privately and confidentially as required by law.
- Exercise the privacy rights described in our Notice of Privacy Practices, including rights concerning access, amendment, confidential communications, restrictions, and certain disclosure information.
- Receive information about practice policies, expected charges, and financial responsibilities applicable to your care.
- Raise concerns or complaints without retaliation or inappropriate impact on your care.
- Request reasonable communication assistance or accommodations when available and required by law.
Your responsibilities as a patient
- Provide accurate and complete information about your health, medications, allergies, symptoms, and relevant medical history.
- Tell your care team when you do not understand instructions, recommendations, or treatment plans.
- Follow agreed-upon treatment plans and discuss barriers or concerns that may prevent you from doing so.
- Keep scheduled appointments or provide notice when you cannot attend, consistent with practice policy.
- Treat staff, providers, other patients, and visitors respectfully.
- Use medications responsibly and follow medication agreements and safety requirements applicable to your treatment.
- Provide accurate insurance and billing information and meet financial responsibilities consistent with practice policy.
- Protect the privacy of other patients and avoid recording others without permission.
